Direct Deposit Form – Landlord request for direct deposit for Housing Assistance Payments

Lead Based Paint Disclosure – Disclosure required for all new move-ins

Contract Rent Increase (CRI) Request Form – Landlord form to request increase in contract rent

Contract Rent Increase (CRI) Dispute Form – Landlord form to dispute an increase in contract rent

Contract Rent Increase Request Schedule – Requests for increases to the contract rent must be received by the RHA at least 60-days before the increase will take effect.

Rent Reasonable Valuation – Property/unit description including amenities used to determine comparable units for rent reasonable valuation

Vendor Change packet – For changes in ownership, property management, or owner address

HAP Contract & Tenancy Addendum – Housing Assistance Payment Contract between the Reno Housing Authority and the owner, plus the addendum to the lease related to the HAP Contract.

Landlord Incentive Request Form for Second Month HAP – Landlords can request an additional month of Housing Assistance Payment if a tenant vacates under certain circumstances (see our Landlord Incentive Program page).

Payment Void/Re-issue Request – Landlords and property managers should use this form to request the RHA void and reissue a previously issued check.

Special Inspection Request – Use this form to request a special HQS inspection of your unit, outside of the normal inspection schedule.

Tenant Forms

Household Member Removal Form (Head, Co-Head and Spouse) – Use this form to report the removal of a household member that is either the Head of Household, Co-Head of Household and Spouse.

Household Member Removal Form (Minor or Other Adult) – Use this form to report the removal of a minor or Other Adult.

Voucher Extension Request Form – Use this form to request an extension to your voucher.

Intent to Move Form – Use this form to report your intent to move to begin the transfer process. Please note that you must provide written notice to your landlord per your lease agreement and supply the RHA with a copy of that notice.

Update Information Form – Use this form to report household updates including but not limited to income and household composition. This form is a fillable document that can be downloaded, completed, saved and e-mailed to section8@renoha.org. Please use your tab key to navigate through the form and your mouse or spacebar to checkmark the boxes. Please answer all questions thoroughly and accurately.

File Review Request Form – Form used to request a file review or hearing

Annual packet – Forms required for all annual re-certification

Simplified Medical Deduction Hardship Request – Form to request hardship for use of MTW simplified medical deduction

Minimum Rent Hardships Forms – Form to request hardship for payment of minimum rent

Guest Request – Form used to request a guest be allowed to stay in your unit for more than 30 days.

Absence From Unit – Form used to request to be absent from your unit for more than 30 days.

Add-a-Member Form – Required to add a member to your household

HAP Contract & Tenancy Addendum – Housing Assistance Payment Contract between the Reno Housing Authority and the owner, plus the addendum to the lease related to the HAP Contract.

Violence Against Women Act (VAWA) Forms – VAWA provides protections for victims of domestic violence, dating violence, sexual assault, or stalking.

Special Inspection Request – Use this form to request a special HQS inspection of your unit. Please attach documentation showing your landlord was notified of the maintenance issues and was given reasonable time to resolve them.

Portability Request Form – Use this form to request that your voucher be transferred (a.k.a. ported) to another area, outside of Washoe County.

Owner HQS Extension Request Form – Form for owners to request an extension for a Housing Quality Standards inspection.

Transportation Waiver – Waiver for the transportation of a minor
